Primary Care First Practice Support offered by KFMC Health Improvement Partners
Primary Care First (PCF) is a five-year payment program based on the principles of the Comprehensive Primary Care Plus (CPC+) model. PCF does not include the tailored practice facilitator support provided to CPC+ practices. KFMC Health Improvement Partners recognizes the value of personalized support to practices participating in PCF, and provides a direct-to-provider, fee-for-service consulting solution. Consultation services can be billed as a fixed annual fee, or by the hour, to meet the needs of all participating practices.
KFMC Health Improvement Partners has been supporting CPC+ practices in the Greater Kansas City Region since the program began in 2017. We are committed to continuing to support practices as they participate in the Primary Care First model across the United States. KFMC has provided learning activities, networking opportunities and technical assistance to CPC+ practices in implementing care delivery changes across all the patients they serve. This knowledge and experience will be shared with practices engaged in PCF. KFMC excels in helping practices leverage data and technology to achieve and sustain higher-performing care delivery processes. Our data-driven insights guide transitions to new, value-based care models, enhanced interoperability and greater security.
